Ohio law requires that the document below be given to policyholders by insurance companies or agents, when policies are issued, as a notice about the coverage available from the Ohio Life & Health Insurance Guaranty Association.  This document is often attached as a policy form or endorsement to life, health, or annuity policies or contracts issued to residents of Ohio.
